How Does Tianeptine Work?

According to recent investigations, tianeptine fully agonistically activates the mu opioid receptor (MOR). [R] [R] The mu-opioid receptors are being researched as potential antidepressant therapy targets, mostly due to their potential as a selective serotonin reuptake enhancer. [R] It is thought that tianeptine’s regulation of these receptors is what causes the therapeutic effects it has shown. [R]

Previous research has linked its effects on the serotonin receptor, dopamine (D2/3) receptors, and glutamate receptors, in addition to its effects on the opioid receptor, as will be addressed below: [R] [R] [R]

Tianeptine has contested the monoaminergic theory of depression as well as the widely accepted monoaminergic processes that have been used to explain the effects of the majority of well-known antidepressants. Tianeptine is a special antidepressant and anxiolytic substance that enhances serotonin (5-hydroxytryptamine; 5-HT) and 5-hydroxy indole acetic acid (5-HIAA) uptake in brain tissue, in addition to the aforementioned processes.

Although serotonin (5-HT), noradrenaline (NA), and dopamine (DA) are monoaminergic neurotransmitters that have been linked to the development of depressive disorders, it is now understood that monoamine deficiencies do not fully account for how antidepressant drugs work. [R]

Tianeptine and Depression

As an antidepressant tianeptine is similar to amitriptyline, imipramine, and fluoxetine, tianeptine exhibits efficacy against major depressive episodes, but with much fewer adverse effects. [R]

In a group of patients with concurrent major depression and anxiety, it was found to be more beneficial than maprotiline. [R]

According to a study, tianeptine exhibits significant anxiolytic properties and is effective in treating a variety of anxiety disorders, including panic disorder. Participants who received 35% CO2 gas (carbogen) while receiving paroxetine or tianeptine therapy exhibited similar panic-blocking effects. [R]

It may also improve cognition in persons with depression-related cognitive dysfunction, just like many antidepressants do (including bupropion, selective serotonin reuptake inhibitors, serotonin-norepinephrine reuptake inhibitors, moclobemide, and countless others). [R]

Tianeptine is beneficial in treating men with erectile dysfunction and depression, according to a 2005 Egyptian study.[R]

Tianeptine and Irritable Bowel Syndrome

In clinical research comparing its effectiveness and tolerability with that of amitriptyline in the management of irritable bowel syndrome, tianeptine was found to be at least as effective and to have less noticeable side effects, such as constipation and dry mouth. [R]

Tianeptine and Asthma

Asthma patients and respiratory depression have shown great success with tianeptine. The findings of a 52-week randomized controlled study of asthmatic children were published in August 1998 by Dr. Fuad Lechin and colleagues at the Central University of Venezuela Institute of Experimental Medicine in Caracas. The children in the groups that received tianeptine had a significantly lower clinical rating and improved lung function. [R]

They discovered a strong, significant correlation between free serotonin levels in plasma and the severity of asthma in those who were exhibiting symptoms two years prior.

They chose to utilize tianeptine to investigate if lowering plasma levels of free serotonin would be beneficial as it was the only medication known to decrease free serotonin and increase serotonin uptake in platelets.

Two double-blind placebo-controlled crossover trials and a seven-year open-label research with less than 25,000 participants both showed efficacy by November 2004. [R]

Tianeptine and Pain

In addition to its analgesic and anticonvulsant properties, tianeptine also has anticonvulsant properties. [R]

A clinical trial conducted in Spain that was completed in January 2007 revealed that tianeptine is beneficial in treating fibromyalgia-related pain. [R]

Tianeptine and Attention Deficit Disorder

The effectiveness of tianeptine in the treatment of attention-deficit hyperactivity disorder has been demonstrated, and its adverse effects are kept to a minimum. [R]
